The Link Between the Acne Drug Isotretinoin and Hair Loss

The Link Between the Acne Drug Isotretinoin (Roaccutane) and Hair Loss

Isotretinoin (Roaccutane, Accutane) and Hair Loss: The Two Faces of an Acne Treatment

"Doctor, I'm taking Roaccutane for my acne, but I'm losing so much hair."

Isotretinoin (sold under brand names such as Roaccutane and Accutane), prescribed by dermatologists, is an effective treatment for severe acne or seborrheic dermatitis that doesn't respond well to other therapies. But behind its effectiveness lies a side effect that shouldn't be overlooked: hair loss. Since the drug is mostly taken by people in their teens and twenties, hair loss that starts after taking it, at such a young age, can cause patients real emotional distress.

1. What Is Isotretinoin?

Isotretinoin is a vitamin A derivative that powerfully suppresses the sebaceous glands, which produce the skin's oil. This dramatically reduces oil production, removes the environment acne bacteria need to thrive, normalizes abnormal skin cell buildup, and reduces inflammation. As potent as it is, side effects such as dry lips, dry skin, and dry eyes are common, and the one we need to focus on here is its effect on hair.

2. Why Does Isotretinoin Cause Hair Loss?

Not everyone who takes isotretinoin loses hair, but in some patients, hair loss or weakened hair can occur through the following mechanisms:

  • Triggering telogen effluvium: Isotretinoin affects the hair growth cycle. It shortens the growth phase, when hair is actively growing, and quickly shifts follicles into the resting phase that comes before shedding, which can cause **telogen effluvium**, a temporary bout of heavy shedding.
  • Dry scalp and follicles: The drug's oil-suppressing action affects the scalp just as much as the face. When the scalp becomes overly dry, it starts to flake, and the healthy environment around the follicles breaks down, leaving hair thinner, dull, and prone to breakage.
  • Reduced follicle cell function: Some studies report that isotretinoin may directly affect the proliferation and function of follicle cells.

Telogen effluvium is known to resolve once you stop taking isotretinoin. The problem lies with patients who already had hair loss, have a family history of it, or have other overlapping causes. For them, isotretinoin can act as the trigger that switches on dormant hair loss genes or accelerates hair loss that was already underway. As a result, hair loss often doesn't stop, or even gets worse, after the medication is discontinued.

3. How to Take Isotretinoin Wisely and Protect Your Hair

So should you give up on this effective acne treatment? Not at all. You just need to use it wisely and manage it proactively to minimize the risk of hair loss.

Dose and Duration Are Key

If possible, start with a low dose and keep the total course short, ideally within 2 months. If you need to take it for longer than 2 months, minimize the dose or space out doses by taking it every 2 or 3 days to prevent the drug from building up excessively in your body.

Self-Care While Taking the Medication

  • Stay well hydrated: Drink more water than usual to counter the body-wide dryness the drug causes.
  • Take hair supplements: A hair supplement containing vitamin C, zinc, iron, vitamin D, and high-quality protein can help.
  • Be gentle with your scalp: Because your scalp can easily become dry and sensitive, use a mild, moisturizing shampoo and apply a scalp moisturizer. In particular, limit dandruff shampoo to once or twice a week.

Isotretinoin is undeniably a good acne treatment. But when you clearly understand its side effects, work closely with a professional to adjust the dose and duration, and actively care for yourself along the way, you can win on both fronts, beating acne and preventing hair loss, and keep both your skin and hair healthy.
